How to enroll on HealthCare.gov: a step-by-step guide for Tennesseans

Last verified · 4 sources · How we verify

The short version: Tennessee uses the federal marketplace, so everything happens at HealthCare.gov (or 1-800-318-2596). Window-shop your ZIP code before you apply, estimate your 2027 income carefully, compare plans after you see your subsidy results, and pay your first premium — coverage doesn't start until you do.

Key dates for 2027 coverage

  • Open enrollment: November 1, 2026 – January 15, 2027
  • Enroll by December 15, 2026 → coverage starts January 1, 2027
  • Enroll December 16, 2026 – January 15, 2027 → coverage starts February 1, 2027
  • Miss the window and you're generally locked out until the next open enrollment, unless a life event (job loss, move, new baby, marriage) qualifies you for a special enrollment period.

The five steps

1. Window-shop before you apply

You don't need an account to browse. Go to HealthCare.gov, enter your ZIP code, and preview plans for your age, household size, and income. This is where you see which carriers actually sell in your area — don't rely on last year's options or TDCI's outdated 2026 map (see Who's selling on Tennessee's 2027 marketplace). Window-shopping first means no surprises later.

2. Create your account and fill out the application

You'll enter your household members and an income estimate for 2027 — not last year's income, your best estimate of what you'll earn in the coverage year. This is the step people get wrong most often, and it matters: your premium tax credits are calculated from this estimate, and the IRS reconciles it against your actual 2027 income when you file taxes. Estimate too low and you may owe money back; estimate too high and you leave help on the table.

A note on verification: HealthCare.gov checks what you report against federal data. Have your documents handy — pay stubs, tax returns, Social Security numbers for everyone applying — and expect the marketplace to ask for proof if your records don't match up.

3. See your results

After you apply, HealthCare.gov shows you:

  • Your premium tax credit amount (if your household income is between 100% and 400% of the federal poverty level — roughly $15,960–$63,840 for a single person, $33,000–$132,000 for a family of four in 2027). Enhanced credits expired at the end of 2025, so the pre-2021 rules and the hard 400% cliff are back. Full detail at Subsidies in Tennessee for 2027.
  • Your cost-sharing reduction (CSR) eligibility (100–250% FPL, Silver plans only) — which lowers your deductible and copays. See Marketplace plan tiers, compared.
  • A referral to TennCare or CoverKids if your income looks Medicaid/CHIP-eligible. That's a good outcome, not a rejection — follow the referral.

4. Compare plans apples to apples

Now you can see real prices after your credits. Compare on four things, not just the premium:

  1. Premium after tax credits — your actual monthly bill.
  2. Deductible and out-of-pocket maximum — your worst-case exposure (capped at $12,000 individual / $24,000 family for 2027).
  3. Network — are your doctors and hospital in-network? Plans can share a carrier name and have different networks.
  4. Formulary — are your prescriptions covered, and at what tier?

Our The Plan Comparison Checklist walks through this side by side. If you're switching from Cigna (exiting 12/31/2026), do this comparison carefully — your doctors' network status may change with a new carrier.

5. Enroll — and pay your first premium

Pick your plan and complete enrollment on HealthCare.gov. Then the step people miss: pay your first premium directly to the insurance company. Your coverage does not take effect until that first payment clears. Set up autopay while you're at it — a missed payment later can end your coverage.

Free help if you want it

  • HealthCare.gov: 1-800-318-2596 — 24/7 except holidays. They can walk you through the application by phone.
  • GetCoveredTenn (Navigator program): 1-866-475-7879 — free, unbiased, in-person help. Funded for 2027; in-person assistance available in Cheatham, Davidson, Fayette, Robertson, Rutherford, Shelby, Sumner, Tipton, Williamson, and Wilson counties, with phone and virtual help statewide.
  • Never pay anyone for enrollment help. Navigators and HealthCare.gov assistance are free; anyone charging you a fee to enroll you is a red flag.

FAQ

Do I have to create an account just to look at plans? No — window-shopping with your ZIP code requires no account. You only create one when you're ready to apply and enroll.

What if my income changes during 2027? Report it to HealthCare.gov as soon as it happens — a raise, a job loss, a new household member. Your credits adjust going forward, and updating promptly avoids an ugly tax-time reconciliation.

I missed December 15. Is it too late? No — you can still enroll through January 15, 2027. Your coverage just starts February 1 instead of January 1. After January 15, you'll need a qualifying life event for a special enrollment period.

Can I enroll by phone instead of online? Yes. Call 1-800-318-2596 and an agent will take your application over the phone, or call GetCoveredTenn at 1-866-475-7879 for free local help.

My application says I might qualify for TennCare. What now? Follow the referral — Tennessee's Medicaid program (TennCare, plus CoverKids for children) will determine your eligibility. If you're approved, that's your coverage; if not, you can come back to the marketplace. Don't ignore the referral and don't buy a marketplace plan in the meantime without checking, since you generally can't get premium tax credits while Medicaid-eligible.
